Efectividad de una intervención enfermera para la adaptación quirúrgica del paciente hipertenso.

Resumen

El paciente hipertenso antes de una intervención quirúrgica manifiesta una ansiedad por la exposición a una situación estresante, que se puede agravar si no tiene una adherencia correcta a su régimen terapéutico. El objetivo principal de este estudio es determinar el efecto de una intervención de enfermería para la adherencia al tratamiento y disminución de la ansiedad sobre pacientes hipertensos que van a ser intervenidos de cirugía ambulatoria en el Hospital de Alta Resolución de Utrera. Se diseña un ensayo clínico aleatorio, con pilotaje previo para consensuar la intervención experimental. La intervención experimental consistirá en una consulta de adherencia al tratamiento antihipertensivo, la entrega de un folleto informativo sobre el circuito quirúrgico y un seguimiento telefónico durante el tiempo de espera. Se establecen un grupo experimental y un grupo control. El nivel de ansiedad previo al acto quirúrgico se medirá con la escala ansiedad estado, el nivel de cumplimiento terapéutico con el test de Morinsky Green y las cifras tensionales con un monitor multiparamétrico. Se utilizará la prueba Chi2 de Pearson y prueba exacta de Fisher para ver si los dos grupos son homogéneos, y la relevancia clínica para ver la eficacia de la intervención experimental. Los resultados buscados en los pacientes son la gestión eficaz de la enfermedad de base y el conocimiento del proceso quirúrgico para evitar posibles complicaciones en su salud.

Abstract

The Hypertensive patients at the ambulatory surgical procedure suffer anxiety by a stressful situation. This could be worse, if the patient haven´t got a correct antihypertensive treatment adherence. The main objective of this study is to determine the effect of nursing activities for medication adherence and decreased anxiety on hypertensive patients who undergo ambulatory surgical procedure at the Hospital de Alta Resolution of Utrera. It’s designed a randomized clinical trial and a prior pilot trial to agree about the experimental intervention. The experimental intervention are a antihypertensive medication adherence office nursing, a surgical patient education handout, a telephone follow-up and a preoperative nursing care the day of surgical. It creates an experimental group and a control group. The level of anxiety prior to surgery measured with state anxiety scale, the level of antihypertensive medication adherence to the test Morinsky Green and the blood pressure with a multiparameter monitor. The Chi2 of Pearson and Fisher exact test was used to test whether the two groups are homogeneous, and clinical relevance to see the effectiveness of the experimental intervention. The results attained by this study are that the patient manages their underlying disease and knowledge of the surgical procedure to avoid health complications.
